Herpes zoster (HZ) is a major complication in patients with autoimmune chronic inflammatory rheumatic diseases, particularly among those receiving glucocorticoids, biologic therapies, and JAK inhibitors. This article reviews current evidence regarding the risk, clinical impact, and prevention of HZ in this vulnerable population.

Available studies show that the recombinant zoster vaccine (RZV) is effective and safe, significantly reducing the incidence of HZ and its complications, even in immunocompromised patients. However, vaccination coverage remains low due to limited access, lack of medical recommendation, and poor risk perception.

The article highlights the need to strengthen medical education, promote vaccination strategies, and develop practical recommendations to optimize HZ prevention and improve quality of life in patients with autoimmune chronic inflammatory rheumatic diseases.
