Dermatomyositis associated with autoantibodies against melanoma differentiation-associated gene 5 (anti-MDA5) is an inflammatory myopathy that frequently presents with hypomyopathic involvement, distinctive mucocutaneous features, and arthritis.

The high incidence of rapidly progressive interstitial lung disease contributes to its poor prognosis, while ulcerations significantly affect quality of life. Hepatic involvement has been reported in these patients, occurring more frequently and with greater severity than in dermatomyositis associated with other autoantibodies.

We present two patients with anti-MDA5 dermatomyositis and hepatic involvement, in whom severe hepatic steatosis was confirmed.
