Atlantoaxial joint involvement in a patient with suspected reactive arthritis. Case report

The case of a 23-year-old female patient is presented, with asymmetrical oligoarticular joint involvement initially that progresses to polyarthritis, along with neck pain of 2 months duration. The immunological laboratory results were negative for anticitrullinated antibodies, rheumatoid factor, antinuclear antibody and HLA-B27; and with positive IgG and IgM results for Chlamydia trachomatis. A x-ray, computed axial tomography and magnetic resonance imaging of the cervical spine and pelvis were performed with evidence of atlantoaxial subluxation and microerosions therein. In addition, significant synovitis was observed in the left coxofemoral joint with subluxation; osteitis or structural changes in the sacroiliac joint were ruled out. The objective of this report is to describe suspected reactive arthritis with involvement of the atlantoaxial joint.
